COVID-19 UPDATE/NOTICE
Due to COVID-19 and for the safety of our patients and staff, we ask parents to wait in their vehicles at the time of their appointments and call our office to check-in. We will give self-check-in instructions over the phone.
Only one parent per family is allowed on the premises. In addition, please inform our staff before arrival if you are experiencing any of the following symptoms: · Fever or chills · Cough · Shortness of breath or difficulty breathing · Fatigue · Muscle or body aches · Headache · New loss of taste or smell · Sore throat · Congestion or runny nose · Nausea or vomiting · Diarrhea Or if you have come in contact with someone with COVID-19 symptoms or have recently traveled. Please log in to your portal account to view appointment details – https://21238.portal.athenahealth.com. Patients coming in for Physicals, please arrive 15 minutes before your appointment time to complete questionnaires. Also, be prepared to show identification and a copy of your child’s insurance card. |
